Overview

To meet the growing needs of the aging population throughout Summit and Wasatch counties, Yarrow Hospice is a new residential hospice agency, located in Summit County, serving families in Summit and Wasatch Counties. Yarrow Hospice has just received its Medicare certification and a three-year accreditation and is ready to assist individuals in the final chapter of life and those who support and love them. We recognize that this journey is wrought with emotional, mental, physical, and often, financial turbulence. It is our mission to ease that turbulence wherever and however possible. “We stand firmly in the knowledge that caring for people during the final transition out of their lives is a profound privilege. It is an honor and deserving of the highest regard and humility,” says Liana Teteberg, President of Yarrow Hospice. As a residential hospice agency, we care for patients and their families in their own home, in any Assisted Living Facility, in skilled nursing homes and anywhere the patient considers to be his/her residence. Yarrow Hospice’s interdisciplinary team is focused on the patient’s comfort and tranquility. To qualify, a physician must certify that the individual has six months or less to live, if the course of illness is allowed to take its natural course. Our services include physician-directed care, professional nursing services, medical social worker and chaplain services. All care and services are built on the patient and family’s needs and priorities and may include hospice aide and homemaker services, physical, occupational and speech & language services in addition to a variety of care as needed. When a family needs assistance, their call will be answered by the owners of Yarrow who live and work in our community. Liana Teteberg has spent a lifetime preparing for this opportunity to provide hospice care to people in need of such services. Teteberg’s clinical experience as a registered nurse and physician assistant was followed by medical administration and management of small and large health care enterprises. Her experience as a consultant provided her with expertise in navigating the complexities of Medicare, Medicaid and the insurance industry that informs Yarrow patients and their families.
