In order to activate your membership, we need the following completed information:

  • Completed Membership Application
  • Copy of your Business License
  • Payment in the form of a check or credit card (download cc form)
    • To calculate your annual dues see Member Dues.
    • Membership dues are based on a rolling 12 months and you will be invoiced 30 days prior to your anniversary date for renewal.
    • Please contact us directly if you are outside Summit County.

You can fax, mail, or email these documents to us or call and set up an appointment to come in and introduce yourself. Our office hours are Monday - Friday from 8am to 5pm. We are located at 1910 Prospector Ave. Park City.
